MDS Coordinator Job Description
Job Summary
Conducts and coordinates the completion of required Resident Assessment instrument (RAI) and is responsible for the implementation and ongoing evaluation of each resident's comprehensive plan of care.
Duties
- Coordination of the facility's RAI process in accordance with state and federal regulations.
- Collaborate with nursing staff and other healthcare professionals to ensure accurate clinical documentation.
- Scheduling and leading interdisciplinary care plan team meetings.
- Auditing for the supporting documentation for all items coded on the MDS.
- Collaborating with health plans, physicians and the interdisciplinary team for assuring insurance approvals.
- Interpreting rules, regulations and coverage guidelines and acting as primary resource for problem solving in regard to the SNF prospective payment system and quality reporting program.
- Comprehensive reviews of Qms and makes suggestions to improve these.
- Confirms transmission files by review and printing of initial and final validation reports.
- Knowledge of case mix scores and CMS regulations desired.
- Other duties as assigned.
Qualifications
- Registered Nurse (RN) with an unencumbered active license to practice in WV.
- 3 years' experience as a registered nurse, one of which includes a supervisory experience.
- Resident Assessment Coordinator (RAC) training must be completed within six months of hire. Preference will be given to those RAC Certified.
- Experience with MDS completion.
