In this role, you will play a vital role in assisting patients with daily living activities and promoting their well-being in the comfort of their homes.
What you'll do:
- Deliver professional nursing care following established care plans.
- Administer medications and treatments as prescribed.
- Monitor and assess the patient’s health, including vital signs and respiratory status.
- Provide mobility support and assistance with daily living activities such as bathing and grooming.
- Collaborate with the patient’s family and healthcare team to create and implement personalized care plans.
- Maintain clear and accurate documentation of care provided.
- Licensed Professional: Current LPN licensure in New Jersey.
- Experienced Caregiver: At least 1 year of nursing experience, preferably with individuals with developmental disabilities.
- Compassionate Communicator: Excellent communication and interpersonal skills.
- Team Player: Ability to work collaboratively as part of a multidisciplinary team.
- Patient-Centered: A genuine passion for improving the lives of others
- Knowledge and experience in handling a ventilator is a plus!
