We are seeking a compassionate and dedicated Community-Based Support Direct Support Provider to join our team and play a vital role in supporting individuals with developmental disabilities!
Your Duties:
● Provide direct care and support to individuals in their homes, neighborhoods, and during community outings, fostering a sense of independence and inclusion.
● Assist individuals with daily living activities such as meal preparation, personal hygiene, medication management (if trained and certified), and household chores, ensuring their well-being.
● Be a champion for community integration! Accompany individuals to social events, recreational activities, volunteer opportunities, and educational classes, expanding their social connections and life experiences.
● Implement positive behavior support plans to promote positive behaviors and enhance individuals' independence and participation in the community.
● Maintain accurate documentation of daily activities, progress notes, incidents, and observations, keeping the team informed and ensuring continuity of care.
● Collaborate effectively with support coordinators, healthcare professionals, family members, and team members to provide holistic support and promote the well-being of individuals with developmental disabilities.
The Ideal Candidate:
● Holds a high school diploma or equivalent; a degree or relevant certifications in human services, psychology, social work, or a related field are a plus.
● Speaks Spanish (Preferred).
● Has previous experience working with individuals with developmental disabilities or in a similar caregiving role (preferred).
● Possesses excellent communication, interpersonal, and collaboration skills.
● Demonstrates a patient, compassionate, and positive attitude.
● Is reliable, dependable, and highly motivated.
