Position Overview : We are seeking a dedicated MSW (Master of Social Work) to join our team and provide Comprehensive InHome Care (CIC) and / or Independent Living Skills Training (ILST) services. The MSW will work with individuals directly to develop and implement personcentered plans that support the individuals goals and preferences and promote independence and community integration.
Responsibilities :
- Conduct initial and ongoing assessments to identify the individuals needspreferences strengths and challenges.
- Develop and implement personcentered plans that address the individuals goalsand preferences and promote independence and community integration.
- Provide ongoing support and coaching to individuals in their homes or communitysettings as specified in the personcentered plan.
- Assist individuals with daily living tasks such as meal preparation medicationmanagement and personal hygiene.
- Provide training to individuals and their families on topics such as budgetinghousehold management and communication. Master Social Worker (MSW) Spanish / English Speaking
- Coordinate with other service providers involved in the individuals care such ashealthcare professionals therapists and case managers.
- Implement and monitor Positive Behavioral Interventions and Supports (PBIS)strategies in collaboration with the individual and their team.
- Provide Independent Living Skills Training (ILST) to individuals to promote their independence and selfsufficiency.
- Maintain accurate and timely documentation of services provided progress towards goals and other relevant information.
- Attend team meetings and other required trainings and meetings. Maintain confidentiality and adhere to ethical and professional standards.
