We are seeking a highly motivated and experienced Clinical Dietitian to join our team. This role offers an excellent opportunity to work with a high-risk senior patient population in a skilled nursing facility environment. You will be part of a very tenured group of dietitians who average 16 years of experience. Additionally, we are willing to train the right individual who meets the educational and licensure requirements but may not have prior experience.

- Flexible Hours Available: We understand the need for flexibility and offer options to adjust your start and end times to best suit your needs. Our core priority is to ensure that our team can support observation of resident meal schedules, with breakfast at 8:00 AM and dinner at 5:00 PM.
- On-Site and
Remote Work:
- On-site Schedule: Contingent upon the Illinois Administrative Code required on-site hours.
- Remote Charting: Opportunity for remote charting; on-site presence required for mandatory meetings with nursing staff for care management .
- No On Call
Rotation
- Location: Assigned Skilled Nursing Facility.
- Note: We are not able to accommodate 10-hour workdays currently.

support to skilled nursing facility residents.

Requirements:
- Bachelor’s
degree from a 4-year college or university.
- Registered
Dietitian with the Commission of Dietetic Registration and Licensed
Dietitian status.
- Previous
experience as a Clinical Dietitian, preferably with high-risk senior
populations in long-term care settings, is preferred but not required. We
are willing to train the right candidate.
- Knowledge of
computer applications (Microsoft Office) and computerized charting
applications.
- Strong verbal
and written communication skills.
- Ability to
write letters, summaries, and reports adhering to professional standards
of punctuation, grammar, diction, and style.
- Proficiency in
mathematical concepts such as probability, statistics, fractions,
percentages, ratios, and proportions.
- Critical
thinking and problem-solving skills.
- Familiarity
with state and federal regulations governing nutritional care in long-term
care facilities?.
Responsibilities:
- Perform
nutritional and professional services to customers under contract with
Prism Health Care Services.
- Maintain
adherence to established professional standards of practice for dietetic professionals.
- Maintain accurate and confidential client records in compliance with
HIPAA regulations
- Serve as a
professional resource for in-service education and provide nutritional
guidance to allied health professionals.
- Function as a
liaison with medical, nursing, social services, dining services, and
administrative staff to develop and ensure adherence to dietetic policies
and procedures.
- Assist allied
professional health workers and food service employees in menu planning
for prescribed therapeutic diets.
- Attend care
conferences as needed.
- Evaluate and
report the adequacy and appropriateness of dietary services with the
facility administrator.
- Record all
nutritional care assessments, findings, and recommendations in the medical
record.
- Attend
regularly scheduled meetings.
- Perform other
duties as assigned.
