Prism Project is a faith-based non-profit that advocates and shelters local child survivors of sex-trafficking. Our 12 year old to 17 year old female residents need a consistent Social Service Worker / Case Manager to help them through our program. Our safe house is located in Genesee County, location is confidential until your background check is cleared.
JOB DESCRIPTION
The Social Service Worker will help with the Participant’s care by assessing treatment needs; developing, monitoring, and evaluating state required service plans and progress; facilitating interdisciplinary approaches; and monitoring Direct Coach documentation logs. The Social Service Worker will also attend weekly team meetings, schedule daily activities, and coordinate off-site transportation for visits.
RESPONSIBILITIES
- Develop and implement behavior intervention plan; monitor behavior intervention plan and meet with youth regarding progress on intervention plan
- Provide restorative mediation support to youth when behavioral concerns need addressed, this includes but is not limited to stress management, de-escalation, crisis prevention, and crisis management and/or transportation.
- Admit new clients by collecting and reviewing records, applications, and conducting orientations.
- Determine clients' requirements by completing intake interviews; determine need for therapeutic, medical, psychosocial, and psychiatric evaluations; and review therapist evaluations, treatment objectives, and plans.
- Establish treatment programs by setting schedules and routines; coordinating services being provided; and arranging resources, including transportation and escort.
- Monitor cases by verifying clients' attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crisis; and providing personal support.
- Maintain clients' records by reviewing case notes and logging events and progress.
- Communicate clients' progress by attending weekly interdisciplinary team meetings and evaluations; disseminating results and obstacles to therapeutic team and family; and identifying treatment influences.
- Prepare clients' discharge by reviewing discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; and providing resources.
- Be able to have a flexible schedule, not limited to evenings and weekends; also rotating on-call as required.
- Maintain CPI certification annually.
- Update job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; and participating in professional organizations.
QUALIFICATIONS
- Bachelor's degree in social work or related field required.
- Two years working in a social work setting, preferably working with youth.
- Computer skills including Microsoft Word and Excel with the ability to learn new programs.
- Ability to solve problems, make decisions, resolve conflicts, and listen.
- Ability to deal calmly in crisis situations.
- Strong interpersonal skills with the ability to be compassionate and firm and always maintain confidentiality.
- Knowledge of community resources.
- Ability to be flexible.
- Ability to work independently as well as part of a team to achieve goals.
- Success at managing a wide array of tasks and projects and an ability to thrive in a fast-paced work environment.

OTHER REQUIREMENTS
- Must have reliable transportation, valid Michigan driver’s license, and valid vehicle insurance.
- Position requires driving in personal vehicle and/or agency vehicle to include freeways and highways to satellite offices, offsite meetings, and events as needed.
- Must be able to work on a flexible schedule (including on-call) as needed to meet the needs of the residents, and deadlines related to job duties, program, and project deadlines.

PHYSICAL REQUIREMENTS
While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds.
