What are the responsibilities and job description for the Case Manager/Companion position at Private family?
This position primarily takes place in Indian Wells, CA, and involves travel to San Francisco, CA from June to October.
Companion/High Level Caregiver
Responsibilities:
- Companion
- Ensure Mrs. remains active and mobile by accompanying her on walks.
- Take Mrs. on drives around town for socialization and recreation.
- Medical
- Maintain awareness of all medical relationships and developments.
- Review and understand weekly test results.
- Aware of general medical condition/vitals and able to discuss at any time.
- Contact doctors, nurses, caregivers, and medical staff as necessary.
- Familiar with medical/care-related policies and procedures for caregivers when caring for Mrs.
- Caregiver Follow-up
- Share medical information from doctors and new orders with the team on the same day as the appointment.
- Notify caregivers of pre-approved new medications and update the medicine box accordingly (after consulting with the manager).
- Motivate caregivers to adhere to Mrs.'s initiatives, set goals, and follow up consistently.
- In the event of staffing issues, capable of performing all caregiver duties.
- Medical Appointment Follow-up
- Take notes during all doctor's visits and email the same day regarding the appointment's details.
- Calendar follow-up appointments made on the same day they are scheduled.
- Medications require prior approval from Mrs.'s doctors before being placed in the medication box.
- Driving Mrs. as Needed
- Holiday Coverage
- Present and active during holidays worked.
- Plan parties for Mrs. and create enjoyable experiences during holidays.
- Party/Event/Social Coverage
- Present and active at parties, events, and social activities.
- Remind Mrs. of names at parties when present with her.
- Ordering/Organizing Medications
- Household Errands as Needed
- Cat Duties
- Record notes from vet-tech appointments and other relevant information about litter box maintenance, food, medicine, and share with the team on the same day.
- Maintain Correspondence for Mrs.
- Schedule thank-you calls and send cards within a week of events.
- Send family birthday cards via FedEx one week prior to the family member's birthday.
- Send Christmas cards via FedEx by December 10th.

- Responsible for personal/family calendaring, coordinating with the team, and other staff.

- Often communicate information on behalf of the family with their friends, family members, and trusted advisors.

- Research, book, logistically coordinate, and oversee end-to-end itinerary preparation for personal travel.
