- PRN Home Care, Inc. is a Medicare certified skilled home health agency. We are looking for a qualified LPN who seeks a close-knit, team oriented, client-centered work environment to join us in caring for Brevard County, Florida residents. Our Home Health nursing staff coordinates comprehensive, interdisciplinary nursing care for patients and empowers patients and their loved ones through education and personal goal setting. We are skilled care and not a companion or block time agency. Duties are a combination of assigned visits as well as opportunity to see more patients on the weekends and assist on-call. Full time and part time positions available for Brevard County, Florida. ASAP!!! You are also welcome to come on in and see us stop by anytime. We are easy to get to off of Murrell Rd. in Viera.
Job Duties/Responsibilities:
*Follow plan of care set forth. We do medication management, wound care, IV infusion, lab draws, teaching etc., per physician orders.
*Ensure care needs are being met with an ongoing focus of goal achievement
*Utilize an interdisciplinary approach to patient care that includes providers and family members
* Connecting patients to the community resources as needed and maintain open and direct communication channels
* Excellent assessment skills
* Superior critical thinking and effective communication skills
* Ability to work in a autonomous environment
* Timeliness with documentation in an EMR system and adherence to organizational standards
* Integrity and self-discipline
Requirements:
* Current Licensed Practical Nurse license in Florida with 1 year acute care experience
* Reliable transportation
* Current CPR
* COVID-19 Vaccination (preferred)
* Skilled Home Care nursing experience preferred but for the right candidate we will be more than willing to train!
