As a home health clinician with PRN HH&T you will visit patients in ALFs and in their homes!

- Holds or can get a current License/certification in Wisconsin.
- Acts as Case Manager and assumes responsibility to coordinate patient care
- Complete and document a thorough assessment of patient and develop individualized patient treatment plans
- Work with clinical staff, patient and families to maximize patient outcomes
- Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
- Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan.
