About Us: PRN Home Health and Therapy is a family-owned company, based out of Appleton and serving 20 counties in central east WI. We offer Skilled Nursing, Physical Therapy, Occupational Therapy, and Speech Therapy. We believe flexibility is important to the success of our employees, since you schedule directly with your patients, you can have a late start, an early start, and even a chunk of time off in the middle of the day!

You are a therapist who:
· Values building connections with their patients.
· Likes working independently while backed by a collaborative team.
· Has great written and oral communication
· Holds or can get a current License/certification in Wisconsin.
Job Duties Include:
· Evaluating and developing individualized patient treatment plans
· Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
· Instruct Certified Occupational Therapist Assistants in treatment procedures to be utilized and provide supervision.
· Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan.
