We’re Looking for Physical Therapist Assistants Who:

- Love working independently, but want to have a team who is always there to support them when needed.
- Are great communicators, both written and orally.
- Want to work part-time or fulltime in home health in the Fond Du Lac and surrounding areaa

Requirements:
- Graduate of an accredited Physical Therapist Assistant program.
- Current license/certification in Wisconsin.
- Current American Heart Association CPR Certification.
Job Duties include:
- Provide Physical Therapy treatments to patients according to treatment plans developed by the Physical Therapist
- Collaborate with the Interdisciplinary Team to identify and assist with obtaining patient specific DME including mobility aides, wheelchairs, cushions, orthotics, and adaptive equipment
- Maintain daily records of treatments and document progress notes on patients as directed by the Physical Therapist
- Collaborate with Physical Therapist and clinical team to develop and educate patient and/or care givers on safe discharge plan
