Pro Care Health Solutions is a rapidly growing company that is looking to add highly professional, self-motivated nurses to join our dynamic team, who love working with the adult and geriatric homebound population in the Macomb, Wayne, Oakland, and Genesee Counties.
MUST be willing to work with in Macomb county (preferably ALL of Macomb County)
Qualifications
- Valid nursing License in good standing with the State of Michigan
- CPR/BLS Certification
- Minimum of one year of nursing experience (2 years preferred).
- Ability to work independently as well as within a team environment
- Exceptional interpersonal & communication skills
- Outstanding organizational and self-management skills
- Exceptional time management skills
- Proficient computer skills
- Up to date auto insurance and valid driver license.
Responsibilities
- Complete initial and ongoing comprehensive assessments of the client’s needs, including but not limited to additional assistance such as physical or occupational therapy, speech therapy, home health aide assistance, and medical social worker to coordinate and/or provide community resources.
- Initiate the plan of care and revise as necessary.
- Supervise care provided by LPN and home health aides, ensuring that services are provided at appropriate time frames and according to the plan of care.
- Assess and chart observations of the patient’s condition at each visit.
- Provide education to patients/caregivers on his/her diagnosis and disease process, medications, pain management, etc. based on the plan of care that is developed with the patient/caregivers.
- Coordinate with the interdisciplinary care team in the patient’s care plan to ensure optimal results.
- Plan for the discharge and/or recert from service (RN responsibility).
- Utilize the Director of Nursing as the first resource when problems or questions occur in the field.
- Maintain Agency property, supplies and equipment in a manner that demonstrates ownership and accountability and recommend tools or devices that may improve the quality of life for the patient.
- Follow-up timely with the physician regarding lab results.
- Demonstrate infection control per Medicare/Joint Commission/OSHA regulations and CDC guidelines.
- Adhere to privacy and security standards provided by all federal, state, and local authorities of the Health Insurance Portability and Accountability Act (HIPAA) and other applicable state laws.
- Provide care, treatment, and services in accordance with professional standards of practice, practice guidelines, law, and regulations.
- Builds community and customer perception of the Agency as a high-quality provider of services.
- Continually analyze work processes and make suggestions for improvement.
- Perform other duties as assigned.

Pay: $65.00 - $110.00 per hour
