Case Manager – Yates OFA: Pro Action’s Yates Office for the Aging (OFA) is seeking an individual to share responsibility for case management functions for the Expanded In-Home Services for the Elderly Program, Caregiver Support, Home Delivered Meal programs.
As part of the team providing programs and services to enhance the quality of life for older adults, the case manager will conduct screenings and intakes for various services provided by OFA. You will also conduct assessments and reassessments of existing customers and monitor changes to their needs, goals, and service options.
Implementing care plans and working collaboratively with other service providers will be part of your daily responsibilities, as will maintaining accurate records and case notes for all consumers and being responsible for timely and accurate data entry, and error free reports.
The successful Case Manager will enjoy working with the older adult population and be good at working collaboratively with others. Must be able to utilize software for accurate data entry and reporting purposes. Excellent verbal and written communication skills are required for this position. The successful applicant will be able to effectively present information and respond to questions from clients and their families, groups, and the public.
Education and experience required for this position include a Bachelor’s degree in human services or nursing or a related field with one year of full time, paid experience or a Registered nurse with one year of satisfactory full-time paid experience. May also possess four years of full-time, paid experience in social casework, social work in a community or social action program, or as a community services worker or case aide in a human services agency.
The ideal candidate must have a driver’s license that meets agency standards and consistent, reliable transportation that will allow regular travel in and around the service area. Agency Driver Record History standards information can be obtained by calling Human Resources at 607-776-2125.
