Job Requirements/Responsibilities

• Current, unencumbered Indiana license.
• Preferred two years of experience in acute or sub-acute care setting.
• Must meet credentialing requirements for Medicare, Medicaid, and Managed Care Organizations.
• Evaluation and management of clients with multiple medically complex health conditions.
• Ability to function autonomously in a fast-paced environment.
• Superior Communication skills.
• Excellent organizational skills and effective time management capabilities.
• Collaborates with facility personnel to ensure multi-disciplinary approach.
• Desire to provide ongoing education to LPNs/RNs on a routine basis.
• Maintains positive and professional attitude and demeanor.
