Hospice RN On-Call Triage

Proactive Hospice Care is seeking a passionate and dedicated Hospice RN On-Call Triage nurse to join our compassionate team. This role is essential in providing after-hours care, addressing patient needs over the phone, managing symptoms, and ensuring the seamless coordination of care. Key Responsibilities:

• Respond to after-hours calls from patients, families, and caregivers to assess needs and provide symptom management.
• Coordinate care with hospice staff and dispatch in-person visits as necessary.
• Ensure accurate documentation of patient interactions, interventions, and outcomes.
• Communicate critical patient information to the interdisciplinary group and on-call physician.
• Provide emotional support and education to patients and their families.
• Patient home visits or death visits conducted overnight will be paid separately in addition to the base monthly rate.
• Phone call forwarding will be provided through the app for ease of communication during on-call hours.

Qualifications:

• Active Registered Nurse (RN) license in Michigan.
• Minimum of 2 years of nursing experience; hospice or palliative care preferred.
• Excellent clinical assessment, triage, and communication skills.
• Ability to work independently and provide compassionate, patient-centered care.
