Proactive MD is seeking a Family Nurse Practitioner to join our Health and Wellness Centers team! Our Health & Wellness Center practice model prioritizes the provider-patient relationship, placing it at the core of the care experience. As a Family Nurse Practitioner within our Centers, you can concentrate your time and efforts on practicing comprehensive medicine, centered on the health and wellness of individuals, following your professional judgment in the most medically suitable manner.
JOB SUMMARY
Proactive MD contracts directly with forward-thinking employers who wish to provide their employees and dependents with high-quality healthcare in the form of a dedicated Provider practice model within their facility. The Family Nurse Practitioner promotes optimum health, prevents illness and injury, and manages acute and chronic health problems for all the plan members.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Oversee/provide patient-centered, comprehensive, primary care services at the Health & Wellness Center including:

Performing episodic, preventive, and/or primary health care

Developing personalized treatment plans in collaboration with patients and Medical Lead Physician

Conducting physical exams, health-risk assessments, and history reviews

Provide referrals to appropriate medical resource or specialty providers as needed

Identify/respond to emergency situations both within Health & Wellness center and elsewhere on the employer premises, within legal scope of practice

Ensures the use of standardized and consistent medical procedures/processes

Participates in the development of annual Health & Wellness Center goals and objectives

Works in tandem with Primary Care Physician, Medical Assistant, Patient Advocate, and Services Director to ensure a collaborative, team-oriented environment

Participate in the development of policies and guidelines (clinical and administrative) to correct and improve service to the client/account

Maintains accurate, timely, and complete Electronic Medical Records

Participates in the incident/occurrence reporting process being mindful of Risk Management and mitigation, quality, compliance and client relationships

Educates and coaches patients

Collaborates with Medical Lead Physician as appropriate

Responsible for clinical oversight and health center consultation regarding administration of health care policy and procedures when Medical Lead Physician is not onsite

Supports, leads and models the vision, mission and values of the company

Other projects and duties as required to address specific medical needs
Qualifications
REQUIRED:
-
Master’s degree (MSN) required
-
Minimum of one-year clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting preferred

Licensed as a Registered Nurse and Nurse Practitioner in state of practice

Nationally Certified as a Family or Adult Nurse Practitioner. Current Certification in AHA or ARC Basic Life support for medical provider is required

Excellent interpersonal skills

Ability to communicate effectively and maintain working relationships with people from diverse backgrounds

Knowledge of the healthcare industry and available patient resources

Ability to prioritize needs and plan work accordingly

Appropriate certification to write prescriptions under the authority of the Medical Leader, if allowed within state of practice
WORK ENVIRONMENT & PHYSICAL REQUIREMENTS
This job primarily operates in a medical office environment. The physical demands to perform the essential functions of this job are:

Effective communication, mobility, ability to operate office equipment and travel intermittently throughout the day.

Must be able to remain in a stationary position at their work area for prolonged periods of time.

Employee will occasionally be required to lift office products and supplies up to 20-30 pounds.
POSITION TYPE & EXPECTED HOURS OF WORK
This is a full-time position, and office hours vary by facility
TRAVEL
Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
