The Strategic Referral Liaison is responsible for coordinating home health care for all patients and referral requests received, acting as a liaison to facilitate communication between referral sources, patients, and health care agencies. The Strategic Referral Liaison maintains accurate records in compliance with applicable laws and regulations and the policies of tango. This role requires excellent interpersonal skills, the ability to handle high-pressure situations, negotiation skills, creative thinking, and a good understanding of the home health industry.
Key Responsibilities:
- Communicate effectively with patients, referral sources, and home health providers to facilitate staffing.
- Maintain accurate and up-to-date records of all referral activities and communications.
- Ensure timely follow-up on all referrals to guarantee patient care continuity.
- Communicate with referral source providers to resolve any issues related to referrals.
- Participate in team meetings and communicate with all parties.
- Escalate clinical needs as necessary.
- Utilize expertise to match patients with the most suitable home health agencies, considering factors such as location, service availability and quality of care.
- Ensure all necessary paperwork and documentation are completed accurately and in a timely manner.
- Notify referral sources of successful staffing of their referrals along with submitting necessary documentation to the receiving home health care agencies.
- Call patients to obtain additional information and notify the patient of outcomes and statuses when necessary.
- Foster collaborative and lucrative relationships with agencies to assist with securing home health services for patients.
- Follow proper procedure to follow up on missing orders or information needed to process request.
- Negotiate terms with home health agency to secure patient care.
- Complete any agency phone outreach to secure patient care
- Will verify PCP and follow up as needed
- Will verify if members want services as needed
- Identify alternatives to home health, when necessary, and communicate to the member and referral source.

Qualifications:
- College degree required.
- Has advanced computer knowledge and experience including Microsoft Word, Excel & typing skills.
- Capable of following an issue through to its conclusion
- Minimum of 2 years of experience in a healthcare setting, preferably in a referral coordination or case management role.
- Strong understanding of medical terminology and healthcare processes.
- Significant experience in negotiation
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficiency in using healthcare management software and electronic health records (EHR).
- Detail-oriented with strong organizational skills.
- Ability to handle sensitive information with confidentiality and professionalism
- Healthcare background is required.
- Is customer service oriented with good oral and written communication skills.
- Is self-directed, flexible, cooperative, and exhibits the ability to work with minimal supervision.
- Is detail oriented and displays good organizational skills.
- Must be flexible and willing to be on call on weekends and Holidays.
- Highly self-motivated and able to work with minimal supervision.
- Must have reliable internet.

- Communicate issues to management and relevant stakeholders.
- Establishes rapport and works with home health agencies to get care for difficult to staff patients secured.
