Health Directions, Inc., a management and consulting company, has an immediate opening for a QUALITY OUTCOME COORDINATOR for the London, Kentucky office. We are looking for a professional who is passionate about improving client outcomes through effective management of home health services.
Job Overview
Responsible for development and coordination of education and training programs tailored to meet the diversified needs of the company and its clients in regard to Outcome and Assessment Information Set (OASIS) outcome measures and processes, and Home Health Value Based Purchasing (HHVCBP) quality measures.
QUALIFICATIONS:
1. Must be a Licensed Registered Nurse or Therapist with experience completing OASIS documentation within a home health care setting. OASIS certification is preferred.
2. Must have a current driver’s license, the ability and willingness to travel and reliable transportation.
3. Position requires the physical ability to perform the job functions outlined under the “Duties and Responsibilities” listed below.
4. Demonstrates expertise in communication, guidance, personnel management and leadership skills.
5. Strong background in educational program development preferred. Must be able to function independently.
