Our Intensive Aftercare Program (IAP) provides services to families where children are at-risk of or have been placed into out-of-home care. As the Residential Treatment / Foster Care Liaison, you'll be working directly with youth who have been placed in residential treatment centers, foster care placements, and / or treatment foster care. Your role will be to ensure that these youth receive the care and support they need to successfully return home. You'll work closely with the youth, the family, and their placement to develop effective treatment plans that address their unique needs and help them achieve their goals.

KEY RESPONSIBILITIES OF THE LIAISON :
- Interview and evaluate clients to complete intake process.
- Analyze client needs and strengths to determine services required to meet treatment needs.
- Assist Residential Treatment Center (RTC) and Treatment Foster Care (TXFC) in formulating and evaluating treatment plan.
- Develop aftercare plan including scheduling reintegration meetings and completing Hillcrest screening.
- Monitor client progress by conducting visits with clients at various institutions a minimum of once per month.
- Maintain monthly contact with client's parents / guardians to provide progress updates.
- Coordinate and facilitate a parent trip to each placement a minimum of once per month.
- Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
- Safely transport clients as needed.
LOCATION : Office is located in Kenosha, WI. Local travel is required (with mileage reimbursement) to meet clients and families at home, residential treatment centers, or other placement settings and community spaces. Must be able to meet in-person with families located in Kenosha County, Wisconsin.
SCHEDULE : Monday-Friday 8 : 00am-5 : 00pm with occasional evenings to accommodate families schedules. Rotating Saturdays are required about once or twice per month.
