Position: Admission/Visit RN
Salary: $68,000 - $80,000 $8,000 Bonus
Schedule: Full-time, Mon-Fri, 8:00 am-4:30 pm with minimal on-call
Coverage Area: Porter, La Porte, Starke, Jasper, Newton, Lake Counties
Benefits: Mileage paid at current IRS rate
Job Duties
Provide and coordinate hospice care for patients at home
Assess patient and family needs, develop care plans
Collaborate with interdisciplinary team
Document assessments and care plans
Requirements
Indiana RN license, Associate degree in Nursing
1 Year Medical/surgical Experience Preferred
CPR certification, valid drivers license, auto insurance
Sensitivity to loss and grief issues required
