RN Case Manager

Must have a valid Registered Nurse license in New Jersey. Responsibilities include:

• Providing case management and discharge planning services to ensure safe, timely discharges and continuity of care.
• Reviewing patient charts within 24 hours of admission for appropriateness and discharge needs.
• Communicating with insurance providers for admission and stay reviews.
• Collaborating with interdisciplinary teams to enhance patient care.
• Coordinating with ancillary departments to decrease length of stay.
  
  

Qualifications include a B.S. in Nursing, active RN license, two years of acute care experience, and preferred case management experience. Strong critical thinking, interpersonal skills, and computer literacy are essential.
