What are the responsibilities and job description for the Certified Substance Abuse Counselor / Case Manager position at Profound Treatment?
Profound Treatment is looking to hire a certified counselor (CCAPP or CAADE) to serve as a counselor / case manager among our team of professionals within our substance use disorder treatment centers.
Duties & Responsibilities
- Oversees coordination of client needs and works in collaboration with client and primary therapist in treatment/discharge planning and goals, identifying additional needed support services including educational, vocational, legal, recreational, mental health and social services
- Completes and maintains all documentation of services delivered
- Ensures cohesion of services documented within client files, and assists with compliance in DHCS and Joint Commission standards
- Keeps a schedule of consistent case management sessions with clients
- Coordinates service provision with probation officers, attorneys, etc., as needed
- Complies with local, state, and federal confidentiality laws and regulations
- Facilitates group counseling sessions
- Collaborates with members of the treatment team to ensure effective delivery of a full spectrum of clinical and social services
- Attends and participates in weekly treatment team meetings
- Participates in staff meetings when appropriate
Skills and Knowledge Required
- Must be able to accurately document all services provided to clients
- Ability to develop excellent rapport with clients
- Must be able to communicate effectively, both orally and in writing
- Must have the ability to coordinate a continuum of treatment / services for clients
Education / Certification Requirements
- Must have a high school diploma or equivalent.
- Must be certified with CAADE, CCAPP or CADTP.
Job Type: Full-time
Pay: $65k-75 annually
Salary : $65,000 - $75,000
