PRIMARY RESPONSIBILITIES:
Under the supervision of the Program Director the Case Manager provides client care and service documentation for clients. Generally, the Case Manager responds to the treatment and rehabilitation needs of assigned clients, monitoring their progress, stability, and movement throughout the course of treatment in the program.
Specifically, the Case Manager must: Assume primary case coordinating responsibilities for all clients that are assigned, including but not limited to advocacy, counseling, crisis intervention, and treatment and discharge planning.
Orient new clients to the Supported Living Program and encourage maximum client participation in treatment. Communicate appropriately with other treatment personnel both within and outside the agency, exchanging information relevant to client treatment. Conduct intake interviews and case conferences, as required to maintain census and coordinate client care. Relay clinical and other client-related or site-related information to other program staff and/or the Program Director at staff meetings and other appropriate times. Provide services in accordance with all State and Federal MediCal requirements and County policies and procedures. Maintain program records including client records, progress notes, problem lists, and other relevant information in accordance with County policies and procedures and State and federal MediCal requirements. Consult with the Program Director as necessary, on such issues as case review, medications, group dynamics, etc. Attend and participate in staff meetings, supervision sessions, and other programmatic and/or administrative meetings, as assigned by the Program Director. Respond to emergencies in a professional, thorough, and timely manner. Monitor the quality and needs of the program physical environments and assist the Program Director in the coordination of necessary maintenance and repairs. Serve as a role model for positive employee relations, professional functioning, and for adherence to the principles of social rehabilitation. Comply with all applicable federal, state and local governmental laws and regulations, as well as all agency policies and procedures, including the code of ethics. Perform other related duties as assigned by the Program Director or the Director of Clinical Services. Inform the Program Director of any personal events and/or circumstances which may affect the ability to perform the proscribed job responsibilities and functions or to meet minimum qualifications.
Qualifications
MINIMUM QUALIFICATIONS:
High school diploma or GED equivalent. Qualifications to meet combined education and experience criteria for credentialing in Napa County as a Mental Health Rehabilitation Specialist preferred. Master's degree in a mental health related field and a social service-related license preferred. Specific experience in residential treatment and/or knowledge of social rehabilitation is preferred. At least two years’ experience in a community mental health program is preferred.
General case management skills, plus good clinical judgment specific to treatment planning, crisis assessment/intervention and milieu therapy are expected. Must be able to communicate effectively with staff, clients, and other providers both orally and in writing. Must demonstrate good judgment, and emotional stability. Must be able to negotiate several flights of stairs during each work shift. Must have a valid California driver license and must be insurable under the agency's current auto insurance policies. Also, must be able to provide own car. Public transportation may be used unless and until, in the sole judgment of management, delays in response time adversely affect the program and/or clients. Must be free of conviction (and remain so) for offenses deemed, in the sole judgement of management, to adversely affect capacity to provide safe and competent services to program clients. At all times must be able to negotiate several flights of stairs multiple times during each work shift and must be able to lift and carry 20 lb. packages multiple times during the work shift.
Salary : $4,900
