plans and implements psycho-education services for IIH participants and their families. Duties and Responsibilities  Establish and maintain positive relationships with other agencies involved in care of participant.  Maintain communication with Program / Clinical Coordinators to ensure the overall well being of participants.  Promote the programs in an ethical and positive manner to elicit referrals and maintain census and growth of the programs.  Submit and maintain an accurate work schedule to be reviewed at any time and submitted to your direct supervisor on a weekly basis.  Arrive on time for all scheduled appointments with participants.  Ensure that every Restraint Form and Incident Report is completed immediately after the incident and submitted to your direct supervisor immediately.  Complete all documentation using sound clinical documentation and abiding by all agency, Medicaid and licensure standards.  Be available for 24 hour Emergency Crisis Response to assigned participants.  Constantly monitor and document participants’ progress towards completion of treatment goals and objectives and for continued eligibility.  Turn in all progress notes within 24 hours of each session and billing / timesheets forms weekly on the assigned date and time given by your direct supervisor. Participate in mandatory supervision (group and individual) on a weekly basis with Program Coordinator, direct supervisor, or Clinical Coordinator.  Complete all monthly reports, quarterly reports, discharge summaries, and Individual Service .Plans on or before the due date.  Attend all mandatory agency training (Handle with Care, CPR & First Aid, Human Rights, HIPAA, etc.)  Accompany participants to and from appointments toward the completion of the treatment goals and objectives on the ISP.  Provide support and linkage to community resources for participants. Complete allocated weekly hours authrized with IIH participants and their families on a weekly basis.  Provide individual psycho-education, crisis intervention, and case management services to eligible IIH participants.  Other duties and responsibilities as assigned. Requirements / Qualifications Be available to provide services during the day, evening, weekends and holidays.  Own an operable, registered and insured vehicle.  Have and use your own vehicle for service delivery.  Keep vehicles registered and insured during the entire length of employment.  Accompany participant and guardian / family to ensure case management and counseling services when deemed necessary.  Provide and maintain an acceptable driving record.  Provide services in various neighborhoods, including crime-ridden and poverty stricken communities.  Conduct home visits and psycho-education sessions within the homes of participants, which may present in unsanitary and / or unhealthy conditions. Possess, keep, use and own a personal cell phone providing the cell phone number to the Program Coordinator, Clinical Coordinator, Field Supervisor, participants, and legal guardians for service delivery.  Possess, keep, and check daily a valid Y-CAPP e-mail address.  Have access to and ability to use a computer, printer, internet services, and other necessary items and equipment to produce electronic program documentation / paperwork.  Maintain communication with Field Supervisor and other Unit employees through e-mail and cell phone usage on a daily basis. EDUCATIONS AND SPECIFICATIONS  Qualified Mental Health Provider (QMHP) within the state of Virginia and (1) possess a bachelor’s degree from an accredited college or university in a human Services related field (ex. Special Education, Counseling, Social Work, Sociology, andPsychology) and have completed at least one year of clinical experience providing direct services to children / adolescents with mental illness.
