As a member of the Progressive Community Services Team, you will commit to the following roles and responsibilities:
· Complete the PCS Support Specialist training program: attend all needed training, fieldwork, and training modules. Work in conjunction with your supervisor to ensure your training needs are met
· Participate in Charting the LifeCourse training to learn about tools and philosophy
· Complete training and keep up to date on an understanding of state systems in the Department of Mental Health (Medicaid Waivers, Medicaid, FFAP, etc.).
· Learn about and utilize exploration tools and assessments to help determine service needs, both initially and ongoing
· Coordinate and monitor the ongoing care of a specialized, non-Medicaid caseload, ensuring health, safety, freedom from abuse/neglect, and the ability to experience one's hopes and dreams
· Caseloads involve a wide range of disability areas and age ranges (approximately 50-60 individuals)
· Build professional and respectful relationships with people you support, the teams surrounding them, community partners, and fellow employees
· Demonstrate sincere and respectful communication (both verbal and in writing) using People First Language
· Work with each person you support to develop a yearly person-centered plan focusing on identifying their goals, interests, relationships, and preferences
· Visit those supported in their home and/or the community at the required intervals for routine planning and monitoring
· Maintain communication with those supported through phone/email
· Link people to needed resources, activities, and connections that can help them achieve their goals and/or explore interests
· Complete visits and all documentation for visits within the required timeframe (quarterly, bi-yearly, yearly, etc.)
· Use software systems to track, log, and maintain documentation while remaining in compliance with agency guidelines
· Work with the Support Coordination team to submit timely service or referral requests as directed by your supervisor
· Assist with team communication and transition if a person supported transfers to Support Coordination with Medicaid services
· Collaborate with Family Navigation regarding resources needed for people supported and/or if a family is transferring to Family Navigation
· Keep current on changes and expectations as they are delivered through team communication
· Engage in department, team, and all staff meetings
· Assist with outreach efforts as directed by your supervisor
· Follow all PCS policies and procedures and keep current on all required training
· Safeguard the security and confidentiality of protected health information per PCS polices
· Demonstrate ability to use and follow best practices for completing job requirements
· Complete all applicable deadlines per the PCS procedures for the Support Specialist
· Work with your supervisor on benchmarks and make improvements where needed
Successful candidates will possess:
· High School Diploma
· Proven ability to build relationships and trust with customers/people supported
· Proven ability to successfully manage your time and set yourself up to work independently
· Ability to attend visits/meetings with people supported in their homes and/or the community as required
· Introductory knowledge about developmental disabilities and the work toward inclusion
· Proficient with Microsoft Office Suite or related software
· Ability to self-direct, manage your timelines, and remote work (if applicable)
Job Type: Full-time
Pay: From $17.75 per hour
