What are the responsibilities and job description for the Direct Support Professional position at Progressive Healthcare Providers - Fern Park LLC?
The Direct Support Professional (DSP) will provide basic care and active treatment to meet the needs of intellectually and developmentally disabled individuals in a home-like setting, in the Fern Park, FL area. We are looking for dedicated and reliable staff to fill several positions in our facility. We currently have multiple shifts open, 7:00am-3:00pm and 3:00pm-11:00pm.
Responsibilities:
- Assists and supports individuals in the implementation of their Individual Support Plans (ISP's) and activity schedules.
- Assists and supports individuals in performing daily living functions.
- Assist and supports individuals in all housekeeping duties to ensure a clean and pleasant living environment.
- Monitor individuals and the surroundings for appropriate safety and health care practices.
- Assist, coordinates and transports individuals to recreational activities in their homes and in the
- Completes documentation regarding programming, health and safety, comprehensive assessments, and
- Monitors individuals and their environment for any change in health, mental well-being, and behavioral well-being.
- Assist and supports individuals in implementing their Behavioral Support Plans as developed.
- Caregiving: 1 year
- Valid FL Driver's License with an acceptable driving record
- High school diploma or equivalent
- Must pass a Level 2 background check, drug screening and TB test
- Must be able to speak, read and write English
- Maintain the ability to stretch, push, pull, kneel, reach, twist, walk or stand up to eight hours and able to perform the physical requirements of Safety Care
PHP/FPDC is an intermediate care facility for the intellectually and developmentally disabled. We care for individuals 24/7 in a set of four (4) homes that hold up to 16 individuals. PHP has excellent opportunities for advancement in the Human Services field. This is a great stepping stone to other positions in Case Management, QA and Administration.
