We are seeking a dedicated and compassionate Mental Health Case Manager to join our team. Duties
- Conduct comprehensive assessments to determine clients' mental health needs and develop individualized care plans.
- Coordinate services and resources for clients, including referrals to community resources, healthcare providers, and support groups.
- Monitor clients' progress and adjust care plans as necessary to ensure optimal outcomes.
- Provide crisis intervention and support during emergencies, utilizing skills in emergency medicine when required.
- Collaborate with multidisciplinary teams to deliver integrated care, including addiction counseling and acute care services.
- Maintain accurate documentation of client interactions, treatment plans, and progress notes in compliance with regulatory standards.
- Advocate for clients' rights and needs within the healthcare system, ensuring they have access to necessary services.
- Engage in ongoing education and training related to mental health practices and case management strategies.
Requirements
- Bachelor's degree in Social Work, Psychology, or a related field; Master's degree preferred.
- Proven experience in case management or mental health counseling is highly desirable.
- Knowledge of emergency medicine practices and pediatrics is a plus.
- Familiarity with utilization management principles and practices.
- Strong communication skills, both verbal and written, with the ability to build rapport with clients from diverse backgrounds.
- Ability to work independently as well as part of a collaborative team environment.
- Compassionate demeanor with a genuine desire to help others navigate their mental health challenges.
- Certification or licensure in relevant fields (e.g., LCSW, LPC) is preferred but not required.
