Mental Health Case Manager

Overview
We are seeking a dedicated and compassionate Mental Health Case Manager to join our team. In this role, you will provide essential support to individuals facing mental health challenges, ensuring they receive the appropriate care and resources necessary for their well-being. The ideal candidate will possess a strong background in social work or mental health counseling, with experience in case management and a commitment to improving the lives of those they serve.

Duties

• Conduct comprehensive assessments to determine clients' mental health needs and develop individualized care plans.
• Coordinate services and resources for clients, including referrals to community resources, healthcare providers, and support groups.
• Monitor clients' progress and adjust care plans as necessary to ensure optimal outcomes.
• Provide crisis intervention and support during emergencies, utilizing skills in emergency medicine when required.
• Collaborate with multidisciplinary teams to deliver integrated care, including addiction counseling and acute care services.
• Maintain accurate documentation of client interactions, treatment plans, and progress notes in compliance with regulatory standards.
• Advocate for clients' rights and needs within the healthcare system, ensuring they have access to necessary services.
• Engage in ongoing education and training related to mental health practices and case management strategies.

Requirements

• Bachelor's degree in Social Work, Psychology, or a related field; Master's degree preferred.
• Proven experience in case management or mental health counseling is highly desirable.
• Knowledge of emergency medicine practices and pediatrics is a plus.
• Familiarity with utilization management principles and practices.
• Strong communication skills, both verbal and written, with the ability to build rapport with clients from diverse backgrounds.
• Ability to work independently as well as part of a collaborative team environment.
• Compassionate demeanor with a genuine desire to help others navigate their mental health challenges.
• Certification or licensure in relevant fields (e.g., LCSW, LPC) is preferred but not required.

Job Types: Part-time, Contract

Pay: $25.00 - $30.00 per hour

Expected hours: 16 – 20 per week

Schedule:

• 8 hour shift
• Day shift
