The Service Coordinator (SC) will manager, supervise and provide direct care services to multiple homes and/or residential support programs.
Some of your Essential Functions will include but not be limited to:
- Supervise and Manage Residential Specialists, services and programs
- Interview and hire Direct Support Professionals (DSP)
- Schedule and complete training to Direct Support Professionals (DSP)
- Weekly home/services billing documentation in accordance with Location Matrix and State/Medicaid requirements
- Review Direct Support Professionals (DSP) weekly timesheets-confirming appropriate staff/persons served ratios
- Attend ISP/IDT meetings, implement ISP plans and provide reports as needed
- Demonstrate the understanding of Progressive Residential Services of New Mexico (PRS-NM) and State DD Waiver standards, policies and procedures
- Monitor a safe and healthy work environment for Persons Served and staff
Must haves, but not limited to:
- Good written and verbal communication skills
- Detail oriented
- Efficient and able to multi-task
- Management skills
- Knowledgeable of trauma informed culture to aid individuals in their recovery process
- Lifting, pushing and pulling of more than 50 lbs
- Walking, bending, kneeling, reaching and standing for full 8 hour shift
- Available by cell phone 24/7 and respond within 15 minutes
Qualifications:
- Experience in Human Services with DD Waiver Persons Served
- 1 year of management experience preferred
- Bachelor's Degree in Human Services field preferred
- English language reading comprehension, speech and written communication skills
- Bi-lingual Spanish preferred
- Proficient with PC computer, including Microsoft Word, Excel and internet
- Valid Driver's License and a Safe Driving Record
- Successful completion of the post-offer process, including criminal history check and all required PRS-NM and state training
