Job Description
This opportunity involves working in our Defiance, OH location, providing in-home therapy services, and collaborating with our Lead Occupational Therapist to ensure seamless patient care.
Key Responsibilities:
- Develop and implement individualized therapy plans for patients
- Conduct in-home therapy sessions, educate patients and caregivers, and document progress
- Communicate effectively with patients, families, and healthcare teams
- Participate in team meetings and contribute to quality improvement initiatives
- Maintain accurate and detailed records
Qualifications:
- Degree in Occupational Therapy from an accredited school
- Current Occupational Therapy Assistant License for the state of Ohio
- Ability to work independently and collaboratively as part of a team
- Excellent communication and interpersonal skills
- Previous home healthcare experience preferred
