About the Role:
We are seeking a compassionate and skilled Physician Assistant (PA) or Nurse Practitioner (NP) to join our team in providing exceptional primary care care to patients in skilled nursing and long-term care facilities. In this role, you will be responsible for rounding for admissions, readmissions, skilled rehab visits, and regulatory visits, ensuring that each patient receives the highest standard of care. This position offers a structured Monday through Friday schedule with no on-call responsibilities, as we have a dedicated on-call team that handles after-hours and weekend care.
Key Responsibilities:
- Provide primary care services to residents in skilled nursing and long-term care facilities in the Grand Junction, CO and surrounding areas
- Patient volume expectations are 15 per day
- Perform regulatory visits, including admission, re-certification, and follow-up visits
- Manage chronic conditions and collaborate with physicians to coordinate care plans for patients
- Maintain accurate and up-to-date patient records
- Ensure compliance with facility policies and healthcare regulations
- Collaborate with nursing staff, social workers, and other healthcare providers to ensure comprehensive patient care
- Provide support for patients and families, addressing questions and concerns as necessary
Qualifications:
- Active and unrestricted Physician Assistant (PA) or Nurse Practitioner (NP) license in Colorado
- Certification as a Physician Assistant (PA-C) or Nurse Practitioner (NP-C)
- Experience in primary care, geriatrics, or skilled nursing/long-term care preferred
- Strong knowledge of regulatory requirements in skilled nursing and long-term care settings
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and patient-centered approach to care
Schedule:
- Full-time, Monday through Friday
- No on-call responsibilities – we have a dedicated on-call team for after-hours and weekends
Compensation & Benefits:
- Salary Range: $100,000 - $120,000 annually
