Position Description

If you are looking for a place where you can make a difference in the life of a child, Project Harmony is looking for you. We are seeking an experienced and committed Mental Health Coordinator to join our dynamic and innovative organization.

The Mental Health Coordinator will work to help children and families access mental health services. The Mental Health Coordinator cultivates referrals, gathers intake information, facilitates mental health and social services for children and families, engages children and families in mental health services and engages partnering providers. The Mental Health Coordinator works with families to remove barriers to mental health services.


Essential Duties and Responsibilities

  • Screen referrals for appropriateness for Connections.
  • Continuously monitor and match children and families to the appropriate network provider.
  • Maintain a weekly summary of matched clients.
  • Complete necessary intake documentation (screenings and assessments) to include demographic information, financial information and basic biopsychosocial information. 
  • Meet with school officials to establish Connections services in schools and maintain a relationship to address issues as they arise.
  • Meet with families and/or providers to facilitate client engagement and to address problems and resolutions as needed.  
  • Maintain communication with families and partners to ensure safe and appropriate services/treatment and to identify any additional needs. 
  • Monitor cases for engagement, problem identification and resolution.
  • Maintain and generate necessary reports and paperwork in a timely manner.
  • Coordinate services in schools requesting groups.
  • Meet with school officials and providers to coordinate the logistics of group.
  • Document information in client management system.
  • Discharge cases when appropriate and send out case close surveys.
  • Provide linkages to families as appropriate.
  • Ensure culturally competent services are provided to all families we serve without regard to race, ethnicity, religion, socioeconomic status, disability, gender or sexual orientation.
  • Participate in designated program and agency-related meetings on a weekly, monthly or quarterly basis to remain informed and make updates and modifications to ensure program effectiveness.
  • Other duties as assigned.

Requirements

Qualifications

Education and Training

  • Bachelor’s degree required in Social Science or related field, but will consider degree in progress. 
  • Spanish-speaking applicants encouraged to apply.

Work Experience

  • Minimum of two (2) years of social work, evidence-based practices, crisis management and/or trauma-informed care experience.
  • Training or experience in case management preferred.
  • Familiarity with all aspects of the Omaha mental health and/or child welfare service system and operation.

Required Knowledge and Skills

  • Knowledge, training and experience in a mental health or closely related child/family program setting.
  • Crisis intervention skills.
  • Family engagement skills.
  • Excellent customer service skills.
  • Must respect and maintain confidentiality.
  • Experience with Microsoft Office, including Word, Excel, and Access.
  • Ability to relate well to a wide variety of individuals, including referral sources, providers, family members, children, etc.
  • Excellent organization, time management and decision-making skills.
  • High level of initiative and energy.
  • Excellent verbal and written communications skills.
  • Good problem-solving skills and ability to resolve conflicts
  • Knowledge of child welfare and mental health community providers.

Other

  • Must be willing to work flexible hours.
  • Must submit to a thorough criminal history background check.
  • Must have access to a vehicle, possess a valid driver’s license and proof of insurance.
  • Position may involve some travel, primarily within the state.
