An integral part of our work is education about the realities of homelessness and poverty and vigorous advocacy on behalf of and with homeless and low-income persons for more just and humane public policies.    Project HOME is committed to nurturing a spirit of community among persons from all walks of life, all of whom have a role to play in making this a more just and compassionate society.    THE VALUES OF PROJECT HOME  The work of Project HOME is rooted in our strong spiritual conviction of the dignity of each person.    We believe that all persons are entitled to decent, affordable housing and access to quality education, employment, and health care.    We believe in the transformational power of building relationships and community as the ultimate answer to the degradation of homelessness and poverty.    We believe that working to end homelessness and poverty enhances the quality of life for everyone in our community.    We believe that the critical resources entrusted to us to achieve our mission must be managed honorably and professionally.  Job Summary The Medical Assistant has the primary responsibility to perform administrative and clinical procedures to ensure all patients seen at the Stephen Klein Wellness Center receive high quality, efficient care.   Essential Duties and Responsibilities   The Medical Assistant is a key member of the primary care team and instrumental to its success. The primary care team includes the patient, his or her family or support system, and the patient’s personal clinician (attending physician, resident physician, or physician assistant, nurse). The primary care team may also include, if warranted, a pharmacist, mental health and/or substance abuse counselor, and/or peer specialist. The Medical Assistant is a multi-skilled member of the health care team who performs administrative and clinical procedures under the supervision of licensed health care providers. The Medical Assistant clinically assesses and triages walk-in patients or in-coming “sick” calls from patients per the Stephen Klein Wellness Center triage protocol. Essential functions and competencies are as follows:     Determine the preferred mode of communication with patient  Demonstrate skills required to provide care to patients.  Assess needs of the patient, providing proper care to meet those needs.  Work effectively within a multi-disciplinary team including providers, nurses, Behavioral Health Consultants, and Community Health Workers  Perform various clinical procedures as required including but not limited to:   Maintain clean and neat examination rooms  Prepare and maintain supplies and equipment for treatments, including sterilization  Prepare exam and treatment rooms with necessary equipment and instruments  Show patients to examination rooms and prepare them for examination and treatment  Interview patients to obtain medical information and take vital signs  Record patients' medical history, vital statistics and information such as test results in medical records.  Ensure all appropriate preventive care is documented in the chart and ordered if due as per protocol  Perform visual screening  Carry out aseptic techniques and follow universal precautions and proper OSHA standards  Provide patient counseling services and patient education consistent with directions of the supervising physician  Collect blood, tissue or other laboratory specimens, log the specimens, and prepare them for testing.  Prepare and administer medications and vaccinations as directed by a Physician, Nurse, Nurse Practitioner, or Physician Assistant   Assist in application of dressings and bandages   Assist in calling in prescriptions to the pharmacy  Performs various administrative tasks as required including but not limited to:  Document and Input all required data into EMR and other relevant databases  Refill prescription medications for patients as per protocol  Retrieve patient voicemail messages from prescription refill line and provider phone line  Retrieve and send clinical faxes  Review provider worklists for lab and imaging results and address as per protocol  Call patients/send letters as directed by the nurse or provider to communicate basic medical information and results  Obtain records of care gaps, recent hospitalizations, emergency visits from hospitals and other healthcare facilities to support the care team and facilitate appropriate follow up    Assist with scheduling of appointments, tests and treatments  Sorting/Handling mail  Medical Records filing/scanning/record-keeping  Assist with submitting medical claim forms patient billing and bookkeeping  Participate in primary care team meetings for case review, quality and discussion of evidence-based chronic disease management, integrated care best practices, etc  Participate in Quality and Risk Management Committee as designated.  Develop and maintain activities to achieve Patient Centered Medical Home as required.  Performs other administrative duties as assigned    Minimum Qualifications   CERTIFICATE/LICENSE-   Graduation from a Medical Assisting training program.   Certified Medical Assistant.   A high school diploma or GED.   Proficiency in Microsoft Office, Excel, internet.   Preferred Qualifications  CPR and First Aid certification preferred.   At least one year of experience as Medical Assistant.    Project HOME is an Equal Opportunity Employer
