Description
Responsibilites:
- Provide care coordination activities for clients.
- Assuring that member has access to, engages in and retains needed services as defined in the members care plan. Such services may include: Acute Medical Care, Primary Medical Care, Preventative medical care Services, Home Health Care, Behavioral Health Services, Community Social Support Services,
- Housing; State and Federal entitlements.
- Advocate for client services by phone and in person as needed.
- Perform home visits and follow-up to monitor services and client status.
- Participate in case conferences and trainings.
- Maintain relationships with service providers.
- Maintain accurate and timely documentation including all documents in client charts, records and billing activity logs.
- Attend supervision with Program Director as per agency policy.
Qualifications
- Requires a Bachelors Degree or a High School Diploma with 2 years of care management experience.
- Experience working with the homeless, and HIV populations required.
- Bilingual preferred, with good verbal skills in Spanish.
- Must have exceptional communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues.
- Clean valid driver's license required.
Salary : $22 - $25
