Description
Responsibilities
- Create tailored and accessible messaging products for DOHMH approval to promote vaccine confidence and/or provide vaccine information.
- Widely disseminate information on COVID-19 vaccines and Long COVID through the weekly distribution of tailored digital messages.
- Conduct an average of 2 in-person street-level outreach activities are completed weekly.
- Host or be the scheduled presenter in virtual or in-person community conversations on a weekly basis with the target constituency related to deepening understanding of vaccine fears/concerns, collecting feedback on vaccine roll-out, building community-driven vaccine confidence and trust, and increasing knowledge of local vaccination locations and availability.
- Provide vaccine navigation services to an average of 100 target individuals per week. Report on the number of navigation engagements, vaccine appointments booked and transportation assists, and other information as relevant.
- Connect community members to Project Hospitality’s continuum of care.
- Participate in funder trainings.

- Qualifications
- Must be bi-lingual in English and Spanish.
- Previous experience as a community health worker or working on vaccine equity strongly preferred.
- Ability to track a variety of data metrics.
