The part-time SCN F/N Specialist will be responsible to complete all required training courses, as identified by agency and Social Care Network SCN) guidelines. This includes but is not limited to: food safety training, various case-management specific training, Motivational Interviewing, Trauma Informed Care, database and referral management training

Staff will be responsible for service tracking and billing/data entry of all rendered services into Channels database.

Staff will be responsible for record keeping, to ensure documentation of rendered services. into the Channels database.

Staff will ensure that all client paperwork and signatures are obtained during service delivery.

Staff will ensure that each client receives the services they are referred for, and note frequency and expiration of services, based on SCN guidelines.

Staff will outreach referred clients to schedule appointments for service delivery and facilitate service completion at both pantry locations.

Staff will prepare the pantry stocking according to program guidelines within the SCN contract.

Staff will coordinate with referral sources (internal and external) in order to discuss referral outcomes as well as communicate any obstacles to client engagement.

Staff will coordinate within the team to provide adequate shopping appointments, inventory check, and communicate any operational issues.

Weekly reports to PD on program progress (number of clients served, types of referrals received, popular food items, successes, challenges).

Staff may be required to assist other program activities, including pantry data tracking/entry, distribution, and inventory management activities.
Qualifications
Bilingual preferred
Minimum HS Diploma/GED
Proficient with computer/internet databases
Salary : $23 - $24
