SUMMARY:
Provides direct care to families assigned to Emergency Hotel Family.
RESPONSIBILITIES:
- Monitors, screens, and oversees all families assigned to the Emergency Hotel.
- Responsible for safety and security of all families assigned to the Emergency Hotel
- Works closely with the case management staff to meet the needs of the families.
- Assists families in developing their ADL skills, housekeeping as needed.
- Transports and accompanies clients to various appointments as needed.
- Engage families on a one on one basis as needed to afford socialization opportunities.
- Monitor families for adherence to project and hotel rules.
- Responds to family health and mental health needs as needed.
- Complete bed roster / bed assignment as needed in DHS-CARES system
Qualifications
This position requires a High School diploma or GED. Experience with the homeless and substance abuse population needed. A clean and valid driver’s license with the ability to drive a passenger van necessary. Ability to establish workload priorities and balance diverse projects. Must have excellent communication skills both orally and in writing with individuals and groups regarding complex and sensitive issues. The physical ability to perform the essential job functions is needed. In addition, the Client Care Coordinator shall perform any duties as required to meet the needs of the agency and the clients that we serve.
