What are the responsibilities and job description for the Social Care Navigator position at Project Hospitality?
Under the direction of the Social Care Network Program Coordinator, the Social Care Navigator is responsible for outreach and engagement of Medicaid members through in person and/or phone screenings to assess health-related social needs, with the goal to facilitate health and social care services to them.
Will be responsible to complete all required training courses, as identified by agency and Social Care Network SCN) guidelines. This includes but is not limited to: various case-management specific training, Motivational Interviewing, Trauma Informed Care, database and referral management training
Manage a caseload of assigned clients.
Conduct outreach, HRSN screenings, and navigation to obtain services.
Maintain and update records of all efforts using appropriate technology platforms, including documenting member eligibility, outreach, contacts, outcomes, and significant statistical data.
Communicate and collaborate closely with other team members, other Project Hospitality programs, partner-based navigators and community partners to obtain and provide information on available services and benefits to help manage member’s specific needs.
Meet with supervisor weekly to report and review outreach efforts and navigation updates, outcomes, and performance.
Ensure compliance with funders’ protocols.
Fieldwork in various settings in Staten Island (community centers, clinics, medical offices, etc.); some weekend hours may be required for outreach events.
All other projects assigned by Social Care Program Coordinator.
Must have a minimum of a HS Diploma/GED.
Must have a valid Driver's license.
Proficient with computer/internet databases.
Bilingual preferred.
Must have exceptional communication skills both orally and in writing with individuals and groups regarding complex and sensitive issues.
