Essential Functions/Responsibilities

  • Will lead Project Kinship in recovery programming for those struggling with substance use disorders and addictions.
  • Will recognize needs and gaps in service for population and will provide feedback to Clinical Coordinator, and program leadership and will collaborate in problem solving.
  • Will build relationships with substance use related service providers in the community, including but not limited to: Detox centers, RTC, PHP, IOP, OP, methadone clinics, MAT, and others.
  • Must be experienced with AA/NA, Abstinence based programs, Harm Reduction & Smart Recovery.
  • Must be a self-starter and comfortable working both independently as well as part of a team.
  • Must be able to build and run recovery-focused programming to include groups, workshops and panels.
  • Conduct initial risk assessment on client’s: physical, mental, emotional, and social needs; skill and literacy level; substance use status; recovery needs; and appropriate levels of care.
  • Conduct individual and small group recovery-focused counseling sessions; workshops on relapse prevention, self-help skills, problem solving, and trigger behavior management with clients and family members; and ongoing case management with clients post release from incarceration.
  • Include clients in their own assessment, evaluation, and treatment plan development to cultivate a sense of responsibility in their own recovery.
  • Work with the multidisciplinary team to create individualized treatment plans which will include short and long-term goals, that are measurable, time bound, reviewed, and updated as needed to successfully progress clients’ treatment plans.
  • Document assessment results, treatment plan updates, group notes, individual session notes, incident reports, and all required documentation in case files in a complete, accurate, and timely manner.

· Document signs and symptoms of use, relapse triggers, and crisis incidents in client’s file and discuss during case consultation with the Clinical Coordinator.

  • Redirect, de-escalate, and intervene during crisis situations, when appropriate; engage emergency professionals immediately when needed.
  • Network with resource agencies to facilitate “warm handoff” linkages to meet treatment goals.
  • Maintain accurate and up to date information in each client’s chart, document client contact by using the EMR (Electronic Medical Record) system, and adhere to confidentiality practices.
  • Maintain an ongoing client caseload of up to 60 clients at any given time.
  • Present cases during weekly treatment plan review meetings with Program Manager and clinical staff.

· Will have a broad understanding of re-entry services and behavioral health services, priorities are to provide mental health services, substance use disorder treatment and diversion programs for individuals in the criminal justice system; provide housing-related assistance, case management, peer support, recovery services, life skills, job skills training and civil legal services.

  • Ensure HIPAA, ethical code of conduct, and professional boundaries are maintained with staff and clients.
  • Participate in trainings, perform assign duties, and submit timecards/leave requests according to deadlines.
  • Implement management decisions in a positive and timely manner.
  • Commit to the agency’s Mission, Vision, and Values; represent actions that define the overall philosophy and objectives of the agency.
  • Work a flexible schedule outside of business hours, including weekends and evening hours, as the need arises, to support the program and leadership.
  • Compile and submit monthly units of service, demographics, and other required evaluation data to the Data Analyst and Director of Programs in a timely manner.
  • And other duties as assigned by agency.

Education and Experience

  • Minimum of High School Diploma or equivalency
  • Certified Substance Use Counselor (CADC, RAD-T, etc.) must be current and able to show proof of degree/certification and minimum of 3 years of experience in substance use counseling.
  • Experience working as part of treatment team in service of dual-diagnosis, gang-involved, incarcerated or formerly incarcerated individuals is highly preferred.
  • Bilingual in Spanish or other threshold languages is preferred.

Other Qualifications

  • Must complete a Live Scan Background Check.
  • Must possess knowledge of: 12 Step recovery curriculums; drug and alcohol addiction; co-occurring substance abuse and mental health disorders; withdrawal processes and symptoms; relapse prevention strategies; trigger behavior identification; family system dynamics; the impact of trauma; and common obstacles clients encounter at all stages of recovery.
  • Able to recognize individuals under the influence, at-risk for relapse, or at-risk for harm to self or others.
  • Ability to work and communicate effectively as part of a treatment team consisting of staff, family members, law enforcement and court officials, service providers, and community members.
  • Extensive knowledge of social service, welfare, legal, corrections, health care, substance abuse, mental health care, and other systems associated with the target population.
  • Must be empathetic, have excellent listening skills, and be able to advocate behalf of the client.
  • Must adhere to policies regarding professional ethics, appropriate boundaries, and client confidentiality.
  • Ability to demonstrate respect for cultural, gender, and lifestyle diversities of clients and staff.
  • Background Check Requirement: The successful candidate for this position will be required to pass or clear two background checks. The first background check will be conducted by Project Kinship as per our policies and procedures. The second background check will be conducted by Funders through our contracted vendors, after the candidate is hired with Project Kinship, in accordance with our contractual agreements. Failure to pass either of these background checks may result in the withdrawal of the job offer or termination of employment.
  • Tuberculosis (TB) Clearance: The successful candidate for this position will be required to provide documentation of a clear TB (tuberculosis) test or shot, in compliance with Project Kinship’s policy. Failure to provide proof of TB clearance may result in the withdrawal of the job offer or termination of employment.
