```Duties:```
- Provide direct care and support to individuals in a residential or assisted living setting.
- Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
- Administer medications and monitor vital signs as directed by healthcare professionals.
- Assist with meal preparation, feeding, and dietary management.
- Provide companionship and emotional support to individuals.
- Assist with light housekeeping duties to maintain a clean and safe environment.
- Transport individuals to appointments, outings, and community activities.
```Requirements:```
- High school diploma or equivalent.
- Previous experience in direct care or a related field is preferred.
- Knowledge of case management principles and practices.
- Ability to perform ADLs and assist with mobility needs.
- Ability to lift and carry heavy objects or individuals as required.
- Familiarity with hospice care principles and practices is a plus.
- Knowledge of HIPAA regulations and ability to maintain confidentiality.
- Strong communication skills and the ability to work effectively in a team environment.
- Valid driver's license with a clean driving record.
