What are the responsibilities and job description for the ISP Coordinator position at Project Redirect?
ISP Coordinator Job Responsibilities
- Oversee, maintain, monitor, and supervise (1) caseload with consists of 30-40 consumers
- Coordinate necessary meetings and have appropriate documentations at meetings (30 day Review, 6th Month Review, Pre ISP, ISP, Case Conference, etc)
- Monitor and track adaptive equipment to report any and all damaged, broken, lost adaptive equipment to DDS Adaptive Equipment Coordinator, DDS Service Coordinator, Residential Provider)
- Ensure that mealtime protocols are being met and notify appropriate persons when appropriate
- Create person-centered long term objectives and goals
- Ensure that contact information for each person is updated an accurate
- Completion of annual documents needed for ISP meetings (Functional Analysis Assessment, PPP, Personal Centered Thinking Tool, LON)
- Monitor and response to MCIS Dashboard Issues
- Creating, distributing, and uploading of quarterly reports according to DDS policy
- Send Reports to Court Reps by assigned due date
- Ensure outside agencies are receiving necessary documents and reports
- Ensure that person’s legal guardians and/or family members are notified about Human Rights Committee Meetings
- Completing of Therap Quarterly Reports in a timely manner
- Use Therap Progress notes to create monthly summaries in quarterly reports
- Checking of ISP amendments daily
- Act as a liaison/advocate for PRD and all waiver services provided by the company
- Print out and monitor Service Authorizations and communication with Accounting Department
- Communicate with appropriate DDS personnel in regard to SA and PA issues
- Print out ISP in timely manner and all supporting documents
- Request and receive, and distribute correspondence for outside providers to appropriate departments
- Ensure annual ISP Training and Emergency Staffing Training and new employee ISP Trainings (Phase II Training) are completed
- Train program staff on person-centered goals and objectives
- Create community outing letters for residential providers
- Sign and approve community outings
- Ensure program staffs are completing daily body observations, monthly attendance, interval documentation. etc.
- Communicate with DDS Service Coordinator in regards to major status changes in goals so that the ISP can be amended
- Act as support staff (spot treatment studios as needed/assist during fire drills)
- Assist in other work tasks as needed
- Attend DDS trainings, presentations, and forums
- Act as a mandatory incident reporter for incidents witnessed, informed, or discovered
- Attend management meetings
- All other PRD policies and procedures, PPD test, CPR first aid AED certification, etc.
- Contribute to and maintain a positive work climate and attitude.
- Contribute talent and skills to the overall team effort
- Perform other duties as assigned
