What are the responsibilities and job description for the Psychiatric Nurse Practitioner (Part-time 16hrs/week) position at Project Transition?

Summary of Job Duties:
Psychiatric Nurse Specialists in an outpatient behavioral health setting works in collaboration with a multidisciplinary team and will practice under the supervision of a supervising Psychiatrist. The supervising physician or a designated substitute physician will be available at all times for consultation. The Psychiatric Nurse Practitioner demonstrates an advanced level of medical and nursing knowledge, clinical/technical competence, sound clinical judgment, professional communication skills, timely and compliant documentation, and is responsible for professional development and competency validation.
Specific Responsibilities:
- Medication Management- preparing for, providing direct service, documenting service. Preparation includes reviewing communication log/attending morning meetings and tx team meetings and gathering pertinent events/history since last visit and recording in EHR so available to the psychiatrist and other tx team members and external stakeholders. Prep also includes the recording of vitals in EHR.
- Monitor physical health needs and interfaces with PCP and external medical providers. This includes monitoring vital signs (weight, height, BMI, pulse, blood pressure, respiratory rate), physical symptoms, medication response and adverse effects, and scheduling appointments as needed in a timely fashion by peer-to-peer discussion and as an appropriate diversion from unnecessary ED usage.
- Send updates to PCP and assist with the transition to THL/OP provider during level 2
- Medication monitoring: MAR, EHR updates, med refills and Prior Authorization forms.
- Health and Wellness Education: group and individual as part of tx plan- includes A and D, smoking cessation, healthy eating/shopping/cooking- tied to incentives, physical activity- also tied to incentives and relaxation skills
- Participate in tx team and DBT groups as well as a community group to keep close to the needs of the members.
- Work closely with the therapist to develop and understand the tx plan and goals in order to be able to model the appropriate interventions in the community at any teachable moment.
- Partner with MD to provide the necessary documentation for quality care as well as for providing medical necessity documentation and support for the level of medical decision making in order to bill Medicare or commercial insurance at an appropriate level.
- Participate in tx team and DBT groups as well as a community group to keep close to the needs of the members.
- Work closely with the therapist to develop and understand the tx plan and goals in order to be able to model the appropriate interventions in the community at any teachable moment.
- Partner with MD on the development of the system for integration of physical health and mental health using telehealth as a conduit for the provision of support.
Qualifications:
- Must have a master’s degree in Nursing with certification from American Nurses Association Credentialing Center (ANCC) as a Clinical Specialist.
- Must possess a certificate of fitness to prescribe, eligible DEA issued by the State of Tennessee Board of Nursing.
- The Psychiatric Clinical Nurse Specialist will maintain licensure as outlined by the North Carolina Board of Medical Examiners for the State of NC.
