Job Summary:
A Community Health Worker (CHW) is a trusted individual who contributes to improved health outcomes in the community and has a close understanding of the community served. The CHW serves as a liaison between health and social services and individuals in the community to facilitate access to services. CHWs serve the communities in which they reside and/or communities with which they share ethnicity, language, socioeconomic status, or life experiences.
Job Responsibilities:
- Conducts outreach to clinics, community centers, emergency departments and other community sites, to find individuals from the target population eligible for enrollment into the Brazos Healthy Communities HUB
- Verifies eligibility and completes all documentation and tools as needed for enrolling the population being served
- Works with enrolled participants in the community and their homes to identify needs and connects them to community based resources and services to remove barriers to care
- Completes intervention plan and case related documentation within the time frame of the established policy
- Ensures participants schedule and attend medical appointments with any providers as needed
- Educates enrolled participants on topics relevant to their situation using the PCHI® Model Learning Modules
- Maintains a full-time active caseload of no greater than 40 low-risk participants and no greater than 30 high-risk participants
- Utilizes Care Coordination System (CCS) software to track and monitor participant progress
- Attends Brazos Healthy Communities HUB and PCHI required trainings and monthly meetings, and attends 2-3 community events/health fairs annually promoting the agency and Brazos Healthy Communities HUB
- Obtains CHW certification (where possible) within one year of hire
- Performs other duties as assigned
Education/ Experience/ Skills:
- Minimum of High School Diploma or GED Equivalent
- One (1) to Three (3) years recent related experience
- Strong interpersonal and communication skills
- Bilingual in English/Spanish is a plus
- General knowledge of medical terminology
- Ability to communicate effectively, function independently, manage time efficiently, organize work flow, and maintain confidentiality
- General knowledge of community resources
- Strong analytical and problem-solving skills
- Strong abilities in understanding data systems and documentation requirements
Job Type: Full-time
Pay: $20.00 per hour
Expected hours: 40 per week
