What are the responsibilities and job description for the Skilled Healthcare Traveler Wanted position at Prokatchers?
Key Responsibilities
- Patient Assessment and Care Planning: Assess patient needs through active listening, social perceptiveness, and analyzing data obtained through direct observation, technology, or health team communication. Create developmentally and age-appropriate, culturally relevant, individualized plans of care to address the nature and scope of identified health problems.
- Care Delivery and Coordination: Provide direct care, monitor patient progress, and coordinate care delivery throughout the patient's episode. Collaborate with the patient, family, physician, and healthcare team members to set goals and adjust the plan based on patient response or change in condition/reprioritization of needs.
- Patient Education and Empowerment: Teach patients and their families what is needed to continue care/healing at home. Assess learning styles and structure sessions to maximize understanding. Ask questions to evaluate retention of key points in the self-care process and provide tools and information to assist the patient's family after discharge.
Requirements
- Experience Required: 1 year experience
- Education Required: Graduate from an accredited school of nursing required
- Preferred Education: Bachelor of Science (or higher) in Nursing Board Approved Program preferred
- Required Certifications & Licensure: Currently licensed by NYS as a registered professional nurse
