Job Summary
ProKure Digital is seeking a skilled and compassionate Nurse Practitioner, licensed in multiple states, to join our team. In this role, you will provide telehealth services while working collaboratively with other healthcare professionals to deliver patient-centered, high-quality care.
Duties and Responsibilities
- Conduct Virtual Assessments and Consultations: Evaluate patients through video or phone consultations, reviewing medical history, medication lists, and patients' current health status.
- Counseling and Education: Educate patients on preventive care, chronic disease management, lifestyle changes, and medication adherence through virtual appointments.
- Coordinate Care and Referrals: Collaborate with specialists, care teams, and community resources to coordinate care, develop effective patient care plans, and ensure proper referrals or in-person follow-up when required.
- Documentation: Maintain accurate and up-to-date medical documentation and patient records in a timely manner.
Experience and Qualifications
The ideal candidate will possess the following qualifications:
- A current, valid Nurse Practitioner license in multiple states
- Proficiency in medical documentation within EMR systems
- A strong background in Primary Care and telehealth medication reviews
- The ability to work both independently and collaboratively within a multidisciplinary team
- Flexibility to work outside regular business hours on occasion
Job Type:
- Part-time, with the potential to transition to full-time based on business needs
- Expected Hours: 15-20 hours per week
