Sat / Sun 9AM-5PM
Setting : Walk In / Urgent Care with Primary Care available
Duties : 72 hours of Face to Face coverage per pay period as sole provider
Days per month : 14-16
Required Procedures : Routine
EMR : Allscripts / Altera / AEHR
Support Staff : Clinic shifts are 1 provider, 1 nurse, 1 front office

- Board Certified - REQUIRED

- Must state availability at name clear (# of shifts per month) - REQUIRED
- Disclose if provider has previously worked at any other CHI / Dignity - REQUIRED
- COVID VACCINATED - REQUIRED
