About the Position
We are seeking an experienced Nurse Practitioner to join our team on a locum basis. As a key member of our medical team, you will have the opportunity to work closely with our Medical Director and Clinic Supervisor to develop and implement clinical policies, procedures, and standards of care.
The successful candidate will be responsible for providing primary medical coverage in collaboration with physician assistants as needed. They will perform initial, semi-annual, and annual assessments, implement appropriate plans of care, and participate in discharge planning for acute and long-term placement.
Responsibilities:
- Provide high-quality patient care in collaboration with the medical team
- Develop and implement individualized care plans
- Conduct regular assessments and evaluations
- Participate in multidisciplinary teams to achieve optimal patient outcomes
Requirements:
- Master's degree in Nursing
- Minimum five years of experience in long-term care
- Active PA License
- BLS, CDS, DEA certifications
- Board Certification by ANCC or AANP
- Certification as a Nurse Practitioner with prescriptive authority
- A minimum of one year working with the frail or elderly population
- Valid driver's license, proof of insurance, and means of transportation
