PROLOGUE JOB DESCRIPTION
Position Title : Case Manager, Homeless Outreach Program
General Description :
The Case Manager provides intensive case management services to clients of the Homeless Outreach Program (HOP). Case managers provide direct client services by establishing goals with clients and then seek to connect services that will assist the clients in reaching identified goals. This person reports to the Program Coordinator or Program Director. Schedule : Full time.
Required Education, Knowledge and Skill Abilities to Perform Essential Job Functions :
A Bachelor's degree is required. Experience in working with special populations is preferred. Must possess the ability to work independently and as part of a team. A good driving record (less than two points) is required. Has the ability to respect individual choice and a commitment to serving homeless populations. This position requires excellent verbal and written communication skills and the ability to manage an assigned caseload.
Summary of Essential Functions, Responsibilities and Duties
- Provide quality intensive case management services to clients
- Work with individuals using a person-centered approach to establish goals
- Use company vehicles to transport clients to community resources and services (food stamps, MA, energy assistance, etc.) and support clients in achieving goals
- Advocate for and support clients in obtaining needed services.
- Provide supervisors with client information in an accurate and timely manner
- Ensure fiscal compliance with agency equipment, vehicles and funds
- Ensure client records are complete and in compliance with all required standards
- Utilize computer systems for external / internal communication, and Time / Attendance on consistent, weekly basis
- Perform other duties as requested by supervisor
