Key Responsibilities :
Nutritional Assessment :
Conduct comprehensive nutritional assessments for newly admitted residents and periodically reassess existing residents to ensure proper nutrition care plans are in place.
Review medical records, dietary preferences, and residents' health conditions to develop personalized nutrition plans.
Care Plan Development :
Collaborate with interdisciplinary team members to develop and modify care plans based on the nutritional needs of each resident, considering their medical diagnoses, eating habits, and cultural preferences.
Ensure all dietary interventions are aligned with residents' individual needs, including those with chronic illnesses, disabilities, or special dietary restrictions (e.g., diabetes, renal, heart disease).
Meal Planning and Menu Review :
Work with kitchen staff and management to ensure menus are nutritionally balanced, diverse, and meet the dietary needs of all residents.
Review and approve menu items for compliance with nutrition and therapeutic requirements.
Ensure that meals are prepared according to specific dietary restrictions and preferences.
Nutritional Education :
Provide nutrition education to residents, families, and staff to promote healthy eating habits and ensure understanding of specific dietary needs.
Conduct group or individual counseling sessions to improve nutritional intake and overall health.
Monitoring and Reporting :
Monitor residents' weight, food intake, and response to therapeutic diets.
Keep accurate records of nutritional assessments, care plans, and progress notes in compliance with facility policies and regulations.
Report any significant changes in residents’ health status or nutrition-related issues to the healthcare team.
Compliance and Regulations :
Ensure that all dietary practices are in compliance with federal, state, and facility regulations, including those related to food safety, dietary standards, and health codes.
Maintain current knowledge of nutrition policies, regulatory guidelines, and best practices.
Qualifications : Education :
Bachelor’s degree in Nutrition, Dietetics, or a related field.
Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certification required.
Licensed Dietitian / Nutritionist (LDN) as required by state law.
Experience :
Previous experience in a skilled nursing facility, long-term care, or healthcare setting preferred.
Experience working with elderly populations and knowledge of medical nutrition therapy for chronic diseases and conditions.
Skills :
Strong knowledge of nutrition principles, therapeutic diets, and foodservice systems.
