Position Overview : We are seeking a compassionate and dedicated Occupational Therapist (OT) to join our team at Atlas Healthcare at Washington Township. The Occupational Therapist will provide skilled therapeutic services to residents in a skilled nursing home, with a focus on helping them regain and enhance their independence in daily living activities. This role requires working closely with a multidisciplinary team to develop and implement personalized treatment plans that meet the physical, cognitive, and emotional needs of residents.
Key Responsibilities :
- Conduct initial and ongoing assessments to determine residents' functional abilities and needs.
- Develop and implement individualized treatment plans aimed at improving residents' daily living skills, mobility, and overall quality of life.
- Provide therapeutic interventions to improve residents’ motor skills, cognitive function, self-care abilities, and adaptive strategies.
- Collaborate with physicians, nurses, physical therapists, and other healthcare professionals to ensure comprehensive care.
- Educate residents and their families on strategies to enhance independence and provide home modifications as necessary.
- Monitor and document residents' progress, adjusting treatment plans as needed.
- Ensure compliance with all regulatory and safety standards.
- Provide training to nursing staff on techniques and equipment that support resident rehabilitation.
- Participate in case conferences and contribute to interdisciplinary care planning.
