What are the responsibilities and job description for the Registered Nurse (RN) for UAS Assessment position at Promenade at University Place?
We are looking for a registered nurse to conduct Initial UAS and reassessment for an assisted living community in Albany NY. Candidates must have at 2 years experience conducting UAS assessments and have good computer skills. This is a per diem assignment for each UAS and related documentation that is completed. The schedule is flexible and based on assessment needs.
- Review physical from primary health provider;
- Address and clarify any questions related to individual physical with primary care provider;
- Determine whether individual is appropriate for assisted living care setting based on community license;
- Conduct initial UAS or reassessment to input individual well-being, evaluation of health status, and care needs; and
- Review such care plan with community wellness team for the development of individualized care service plans.
Requirements:
- Current and Valid NYS RN License
- Excellent Written and Verbal Communication Skills
- Minimum 2 years nursing Experience
- Excellent Patient Care and Assessment Skills
- Experience with UAS Assessment / documentation
